The purpose of this study was to determine the effectiveness of video-guided exercise programs on function in frail nursing home residents. Sixty-eight elderly dependent in at least two ADL residing in the nursing home for at least 3 months participated. Subjects ranged in age from 71 to 97 years. Fifty-four patients participated in the video-guided exercise programs, 14 patients served as controls. Five residential care facilities in the MetroWest area Massachusetts were used.

Three exercise levels were recorded on video-tape for the study: Exercise for Ambulatory, Non-Ambulatory, and Bed-ridden Elders. A fourth tape was produced to standardize staff training in the exercise protocols. Patients were assigned to exercise groups dependent of functional level as determined by the Physical Disability Index. Controls were assigned based on their agreement for involvement in evaluative sessions, but refusal to participate in exercise. Exercises were done three times a week for four months including: range-of-motion, flexibility, trunk and extremity strengthening, balance, and mobility exercises.

Performance-based physical function was assessed by Physical Disability Index; self-perceived health status assessed with Sickness Impact Profile; observer-reported ADL utilizing the Katz Functional Assessment Scale; and monitoring falls.

The exercisers experienced no significant improvement in overall Physical Disability Index, Sickness Impact Profile, or activities of daily living scores. A 15.5% improvement in the mobility subscale of the Physical Disability Index was seen. The exercisers in Ambulatory and Non-ambulatory groups used assistive devices for bed mobility and transfer tasks less often and were less likely to use wheelchairs for locomotion. There were 79 falls in the exercising groups versus 60 falls in the control group. Charge for the 4-month exercise program was $1,220 per subject.
