Prior research has demonstrated the role of cancer knowledge and beliefs in the use of certain health services, such as cancer screenings. This dissertation sought to understand how older, rural women from an underserved area of Appalachian Kentucky (Estill County) develop knowledge and beliefs about cancer in general and breast cancer in particular, and how beliefs shape the use of cancer screening. Fifteen women ages 65 and older without a history of cancer diagnosis were interviewed for approximately six hours across multiple sessions. Questions assessed the perceived availability of health and cancer resources, health and cancer beliefs, knowledge of general and breast cancer and screenings, and actual cancer screening behaviors across each woman's life course. After full transcription, each interview underwent coding and thematic analysis using NVivo qualitative analysis software.

The women in this study all understood the importance of detecting cancer in the earliest stages in order to increase chances of survival, but they hold on to certain beliefs (i.e., cancer equals death, cancer is incurable, air makes cancer spread, treatment is worse than the disease) that run contrary to wanting to actually find cancer. However, having these beliefs does not seem to matter in the big picture of breast cancer screening because all of the women reported having yearly mammograms. The role of beliefs appears to be larger than only encompassing health or disease-specific beliefs; in the emerged model in this study, multiple belief domains (including health, risk-taking, and religiosity/spirituality) were presented as a different way of understanding health behaviors and health service use. Understanding why women engage in cancer screenings despite having these strong, negative beliefs of cancer and its treatment is an important step forward in this area of research.
