The incidence of HIV/AIDS among people over the age of 50 years is reported to be 10 percent with a prevalence rate of 12--15 percent. The exact numbers of people 50 years and older who have HIV is presently unknown as this population is less likely to be tested until the person develops AIDS-related diseases. Health policies aimed at providing education and screening for this age group have been virtually non-existent. The elderly in general, do not see themselves as being at risk for HIV, do not practice safe sex and do not recognize symptoms of HIV infection. Health professionals in general fail to impart information about HIV risk to older patients and neglect to identify dementia and immune deficiency diseases as related to HIV, tending instead to attribute symptoms to disorders of aging, rather than to AIDS.

The principal goal of this research is to examine how health professionals (physicians and nurse practitioners) impart information about HIV risk to older patients. It will examine the manner in which HIV information is shared as well as perceived barriers to all parties which prevent the diffusion a dialogue about HIV transmission, risk and prevention.

This dissertation is a qualitative examination using a survey tool that was developed to determine the knowledge level and beliefs regarding HIV of low income blacks over the age of 65 years living in two zip codes in Philadelphia. A second tool was developed to examine the degree to which health care providers recognized the risk of HIV among older people and how information about this risk is disseminated to patients.

Data was collected using a combination of structured interviews and open-ended questions with older people and health care providers in the University City section of Philadelphia in order to gain an understanding of the knowledge of HIV/AIDS risk in those over 65 and to formulate policy initiatives to address the needs of this population with regard to HIV prevention. The results indicated that neither physicians nor nurse practitioners in this group routinely raised the issue of HIV prevention with their patients. Moreover, the older adults were aware of the issues of HIV in their communities but assumed that their physicians would inform them if they tested HIV positive based upon a belief that HIV testing could be performed without the expressed written consent of patients in Pennsylvania at the time this study was conducted.

There clearly exists a need for health providers to discuss the subject of HIV prevention with their older adult patients, to provide information about testing and to disseminate information about HIV among this population.
