Recent initiatives in care for older persons with disabilities include geriatric rehabilitation units (GRUs). Transitions into and out of this health care setting involve both the patient and his or her family members. Even though many older adults transitioning from a GRU to home are doing so with at least one family caregiver, caregivers' experiences of the actual transition of their relative from a hospital-based setting to home and the processes engaged in by families has received less attention from researchers. The purpose of the present study was to develop a theoretical framework that advances a deeper understanding of spousal caregivers' experiences and the processes in which they engaged during their husbands'/wives' transition from a geriatric rehabilitation unit (GRU) to home. A constructivist grounded theory methodology was employed. I interviewed 18 spousal caregivers whose husbands/wives were transitioning from a GRU to home. Forty-five interviews took place across 3 points in time: approximately 2 days prior to discharge from the GRU, 2 weeks post-discharge and 1 month post-discharge from the GRU. A theoretical framework was developed within which Reconciling in response to fluctuating needs emerged as the core process. The framework highlights the ways in which caregivers responded to the fluctuating medical, physical, emotional and social needs of their husbands/wives, for themselves and for the marital dyad by reconciling through three phases: Getting Ready, Getting Into It and Getting On With It. Reconciling is situated within a larger context of both the trajectory of care transitions experienced by caregivers alongside their relatives and intertwined life events. The theoretical framework serves as a resource to increase understanding for health care scientists, rehabilitation clinicians, educators and decision makers towards improving transitional care experiences for spousal caregivers.
