The beneficial and deleterious effects of grief on the health and well-being of bereaved individuals have been well established in past research. While much of this research has focused on the grief reactions of family members, far fewer studies have investigated the experiences of healthcare professionals following the deaths of patients. The purpose of the present study was twofold: (1) to investigate the grief experiences of certified nursing assistants (CNAs) in the nursing home setting and to identify and evaluate the factors that mediate or exacerbate grief; and, (2) to examine the impact of these grief experiences on feelings of burnout and the desire to leave either the nursing home or the nursing profession (i.e., potential for turnover).

A cross-sectional, quantitative design was employed, with participants completing self-administered surveys (N = 136). A series of multiple linear regression models were constructed to identify significant predictor variables. Results confirmed that CNAs can experience significant grief reactions to the deaths of nursing home residents under their care. Several types of factors were found to influence grief, including environmental factors, such as employment in urban facilities, personal factors, such as fear of death, and professional factors, such as organizational grief support. Positive grief outcomes, such as personal growth, were found to play a prophylactic role in preventing burnout, while negative grief outcomes, such as complicated grief, were found to contribute to burnout. However, the grief experiences of CNAs were not found to have a significant relationship with potential for turnover, indicating that the relationship between grief and potential for turnover may be less direct and more complex than anticipated.

Practitioners and researchers may benefit from the results of this study through the construction and evaluation of interventions aimed at accentuating the positive outcomes and alleviating the negative outcomes of grief for this group of important healthcare professionals. Finally, nursing home residents and their family members may reap benefits as we gain insight into the effects of grief on the lives of CNAs and, ultimately, on the care that they provide to the growing number of older adults in our society.
