Americans are living longer and enjoying better physical health in their retirement years than ever before; however, the nation is failing to address the growing mental health needs of its older citizens (Mental Health and Aging, 2001). On the whole, the elderly report many positive exchanges and relatively fewer negative ones. However, in spite of the small number of negative interactions reported by the elderly in studies, findings consistently demonstrate the potent effects of those negative interactions on the well-being of the elderly (Finch, Okun, Barrera, Zautra & Reich, 1989; Rook, 1984).

The ability to develop and maintain strong relationships and social support systems have been identified as important aspects of how well individuals age (Rowe & Kahn, 1998). It is also important to mention that solitude, or a lack of social interaction, is considered a major health risk factor (Unger, McAvay, Bruce, & Seeman, 1999). This is an ongoing risk for elderly individuals who live in a rural environment. These individuals often experience difficulty with service access, service utilization, as well as, a lack in social interaction.

Rural elderly are among the most at-risk populations for mental health problems (Wagenfield, 1990; Buckwalter, Abraham, Smith, & Smullen, 1993), and report more physical illnesses, physical impairments, and disabilities than their urban counterparts deeper understanding of the mental health issues of rural elderly who attend an Intensive Outpatient Program in Bernice, Louisiana.

This study utilized a qualitative ethnographic design to answer two main research questions. The data generated from the interviews was analyzed for emerging themes through pattern finding.

The results of this study indicated that there are four major themes that contribute to rural, older adults entering an Intensive Outpatient Program for mental health treatment. These themes are: physical illness, memory problems, grief, and family difficulties. As a means of coping with these themes, participants indicated that they often take part in social outlets and they communicate their thoughts to group members, therapists, friends, and church members. On the other hand, negative interactions with family members influence symptomatic behaviors.
