This research sought to examine executive function in a medication management assessment tool, the Pillbox Test. Interest was in examining practical aspects of executive function and score patterns that differentiate clinical groups. Specific interest was in the geriatric patients with cognitive disorders. Community hospital outpatients were evaluated for various reasons including information about their ability to manage medications. Patients with cognitive disorders were thought to be at risk for medication adherence difficulties.

Patients were administered a comprehensive neuropsychological battery, which included executive function tools, and real-life measures of problem solving such as medication management. Focus was on select executive function measures, cognitive tests, and the Pillbox test scores. The Pillbox Test is a performance measure that relies on demonstration rather than self-report of skills. Pillbox scores were recorded as a variety of variables including total number of errors, and error patterns.

The Pillbox Test demonstrated a statistically significant difference to identify between clinical groups, and had positive correlations with established executive function measures. Medication management scores are useful in combination with other psychometric tests to diagnose cognitive disorders, and are a stand-alone tool to examine medication management across select clinical populations. Clinical significance was examined by charting Pillbox error patterns across clinical groups. These findings support the directions in test development: the development of ecologically valid assessment tools, and functional outcome measures that focus on performance aspects of diseases. Directions for future research are discussed that include various uses and modifications of the Pillbox Test as a diagnostic, teaching, and evaluation tool.
