The new population which resulted from the dramatic increase in the cancer survival rate led to the awareness of the emerging phenomenon of cancer survivorship. Constructivist Grounded Theory methodology with 18 women, 70 years and above with diverse sociodemographic circumstances who had recently completed treatment for primary, early stage breast cancer, allowed three distinct categories to emerged from the data. The first category reflected a trio of unique barriers to care encountered by the women from the period before diagnosis through treatment and beyond. The three barriers were misinformation and/or lack of knowledge, pre-existing comorbid diseases, and multiple appointments with healthcare providers. Oncology nurse navigators were remembered as particularly helpful throughout this process. The second category, called "instrumental relating", focused on the women's ways of relating to others with intention and purpose to navigate a path riddled with impediments in order to make decisions about their treatment. The women spontaneously and directly noted when they received care from female physicians, particularly if the doctor had also received treatment for breast cancer. The third category involved the concept of cancer survivorship as perceived by the women themselves. The women in this study did not personally identify with the concept of cancer survivorship as the public term is presently understood. Some women had never heard of the term. Cancer survivorship was not a discrete concept to this group of women, instead they described a Cancer Survivorship Spectrum. Implications for clinical practice and future research are described.
