摘要註:	台灣於2007年提出遠距健康照護先導型計畫，並建立了遠距健康照護平台，以連結居家、社區與機構等三種不同的照護模式，迄今已近10年的推廣經驗，但仍未有成功的商業模式，為了解國內外推廣遠距健康照護經驗。本文希望透過文獻回顧探討方式，探討國內外遠距健康照護推動遭遇的挑戰與關鍵成功因素，做為未來推動遠距健康照護的參考。本研究之資料來源有4個電子資料庫，中文期刊以CEPS為主，英文期刊則以SDOS、Medline、Google學術搜尋為主，中英文關鍵字為遠距健康照護、成功關鍵因素、遭遇的困難、Telehealth、Key Successful Factor、obstacle，搜尋2000年1月至2016年12月發表之論文，共146篇，其中篩選出12篇符合遠距健康照護和成功關鍵因素、遭遇的困難之國內外研究。綜合國內外研究結果，經整理及歸納分析，遠距健康照護遭遇的挑戰與關鍵成功因素有以下10項要點:（1）法令與政策；（2）民眾需求；（3）組織支持；（4）系統品質與便利性；（5）民眾健康管理意識；（6）倫理議題；（7）民眾就醫習慣；（8）經濟條件；（9）系統資訊互通與標準化；（10）以病人為中心；(11) 提供連續性的服務；(12) 地理環境限制。最後依據上述關鍵成功因素，提出未來推動遠距健康照護的建議。

In the process of population aging, we focused on the study of the obstacles and key successful factors of telehealth, hoping to understand the current situation of telehealth at home and abroad through the literature review, as a reference for Taiwan's future medical institutions, community and long-term care institutions in the elderly clinical care. The fulltexts of this study were from four electronic databases. They were CEPS, SDOS, Medline, and Google Scholar. Chinese and English keywords were telehealth, obstacles, and key successful factors which were from January 2000 to December 2016 domestic and foreign journals, a total of 146, which selected 12 corresponding to the topics which were related to telehealth, obstacles, and key successful factors. Based on the results of the research at home and abroad, the key factors that showed the challenges and successes were related to the following twelve dimensions:1. Policy. 2. Public demand. 3. Organization support. 4. The quality and convenience of the telehealth system. 5. Public awareness and health management awareness. 6. Ethical issues. 7. People's medical habits. 8. Economic issues. 9. Interoperable and standardized information system. 10. Patient-centered. 11. Providing continuous care system. 12. Geographical restrictions. Finally, based on the above key dimensions, some suggestions for developing telehealth care system in the future have been proposed.
