摘要註:	隨著醫療科技進步，平均壽命不斷延長，年老者的身體機能日漸退化，不僅影響日常生活，更可能失去行動能力，因此長照機構專業照顧服務的需求將日漸飆升，卻也面臨不知如何讓長輩得到更好的照顧，思考讓長輩得到更好的照顧服務是當前重要的課題。本研究利用質性研究法之半結構式的訪談來收集資料，針對5間長照機構管理者在導入自立支援照顧後之執行狀況進行訪談，經彙整及分析訪談資料後歸納得知，機構導入自立支援照顧之實施前，舉辦會議取得共識有其必要性，積極參與銀髮體驗課程讓其產生同理心，機構應做為後盾協助解決困難，各類專業人力無須增加，僅需適當調整工作內容，採相互支援之方式執行，環境設備視現況需求調整，對於營運成本影響不大，在不約束、不尿布、不臥床之三不照顧原則的部分，以循序漸進減少三不的模式進行，讓工作人員逐漸找到成就感，吸引更多年輕學子的投入，至於照顧計畫應評估住民本身之狀況及希望達成的目標做擬定，當長輩自主生活能力提升了，對機構本身就是最佳的宣傳，藉由研究結果期望能提供機構在導入自立支援照顧時之參考，未來長照機構推展自立支援照顧模式將成為主流，政府應以政策引導機構之全面執行共識，提升照顧現場正向之整體氛圍，讓長輩過著有尊嚴的晚年。

The physical function of the elderly is deteriorating, which not only affects daily life, but also may lose mobility. Therefore, the demand for professional care services of the long-term institutions will gradually increase, but they also face the problem of how to let the elders get better care. Thinking about getting better care for the elders is an important issue at the moment. This study used semi-structured interviews of qualitative research methods to collect data, and interviewed the manager of five parental care institutions after the introduction of self-supporting care. After the integration and analysis of the interview data, it was concluded that it is necessary to hold a meeting to reach consensus before the institution introduces self-support care. Actively participate in the experience course of the seniors to create empathy. The organization should be used as a backing to help solve problems. There is no need to increase the professional manpower of all kinds. It is only necessary to adjust the work content appropriately and execute by mutual support. The environmental equipment is adjusted according to current situation, and has little impact on the operating cost. In the part of non-constrained, non-diapering, and ambulatory, gradually reduction of the three is carried out. Let the staff gradually find a sense of accomplishment and attract more young students’ input. As for the care plan, it should assess the status of the residents themselves and the goals they hope to achieve. When the elders’ self-reliance ability is improved, It is the best publicity for the organization itself. The results of the study are expected to provide reference for the organization to introduce self-supporting care. In the future the long-term institution’s self-supporting and supportive care model will become the mainstream. The government should guide the full implementation of the consensus by the policy and improve the overall atmosphere of the front of the care site. Let the elders live a dignified old age。
