摘要註:	台灣已於2018年3月正式邁入高齡社會，預估於2026年進入超高齡社會，隨著社會人口結構改變，失能者的照顧需求將隨之增加，為實踐「維持生活獨立、延緩失能、減少照顧支出」的長照政策核心目標，自立支援照顧模式從日常生活喝吃拉動基本照顧中實踐，提倡「不約束、不尿布、不臥床」的照顧原則，徹底顛覆傳統的照顧觀念，將成為未來主流照顧模式，本研究採用質性研究之半結構式訪談大綱，針對五位於日間照顧中心實施自立支援照顧模式的個案進行深度訪談，探討個案在日間照顧中心接受自立支援照顧模式之經驗感受，並分析自立支援照顧模式對個案的照顧成效與影響因素，彙整訪談分析結論如下：（1）自立支援照顧模式體現日間照顧中心設置的目的；（2）驗證了實施自立支援照顧模式激起失能或失智個案的動機與落實基本照顧的重要性；（3）了解影響自立支援照顧模式成效的助力與阻礙因素；（4）由個案的感受回饋更顯自立支援照顧模式推廣的必要性﹔本研究結果可提供「自立支照顧模式」於日照中心、社區據點推廣的參考，以降低高齡社會的照護成本及負擔，讓長輩能健康幸福的在地老化。

Taiwan is going to be an super-aged society in 2026.As the demographic structure of society changes, the need of care for the disabled elderly will be gradually increasing.The main care model in the future-Self-supporting care model,overturn the traditional concept of care,would follow the implementation of the core goal of the long-term care policy,which is "maintaining independence in life, delaying disability, and reducing care expenditure".The is practiced from the basic care of drinking, eating, excretion and exercise in daily life.Promoting the principle of "unconstrained, non-diaper, ,not stay in bed ".It approached a semi-structured interview method and conducted in-depth interviews on five cases who implemented Self-supporting Care Mode in day care center. The research explored that the experience and feelings of the case of the self-supporting care model in the day care center.and analyze the effectiveness and influencing factors of the model of them.The conclusions of the interview analysis tha is as follows:The self-supporting care model could prove the purpose of the day care center setting; It is important to stimulate motivation and implement basic care for disabled or dementia cases;Understand the advantage and disadvantage; Get feedback from the individual's feelings and the need. The results of this study provided a "Self-supporting care model" reference for the daycare center or communities, reduced the cost and burden of social care for the elderly, and enabled the elders to age healthily and happily.
