摘要註:	本研究旨在運用健康信念模式（Health Belief Model, HBM）探討南投縣中高齡者對慢性病自我管理之影響因素。隨著台灣人口高齡化，慢性疾病盛行率逐年上升，中高齡者的健康行為與自我管理能力成為公共衛生推動的重要課題。健康信念模式強調個體對疾病威脅的認知及行為改變的動機，適用於探討影響健康行為的心理因素。本研究採橫斷式問卷調查法，以南投縣年齡45歲以上、罹患慢性病之中高齡者為研究對象，透過結構式問卷蒐集資料。研究工具包含健康信念模式量表、慢性病自我管理行為量表及基本資料問卷。資料分析採用描述性統計、皮爾森相關分析與多元迴歸分析。研究結果顯示，健康信念模式各構面與慢性病自我管理行為之間具有顯著相關性，其中知覺利益與自我效能對自我管理行為具正向影響，而知覺障礙則具負向影響。不同背景變項，比如教育程度與疾病種類，在健康信念與自我管理行為上亦存在顯著差異。研究結果可作為衛生單位與社區關懷據點規劃中高齡健康促進課程之參考，並驗證健康信念模式於慢性病自我管理之理論適用性。

As Taiwan's population ages, the prevalence of chronic diseases rises annually, making the health behaviors and self-management capabilities of middle-aged and older adults a critical issue in public health promotion. The HBM emphasizes individuals' perceptions of disease threats and their motivation for behavioral change, making it highly suitable for examining the psychological factors that influence health behaviors. Employing a cross-sectional questionnaire survey, this study focused on middle-aged and older adults aged 45 and above with chronic diseases in Nantou County. Data were collected using a structured questionnaire, which included an HBM scale, a chronic disease self-management behavior scale, and a demographic questionnaire. Data analysis involved descriptive statistics, Pearson correlation analysis, and multiple regression analysis. The results indicate significant correlations between HBM dimensions and chronic disease self-management behaviors. Specifically, perceived benefits and self-efficacy positively influence self-management behaviors, whereas perceived barriers exhibit a negative impact. Furthermore, significant differences in health beliefs and self-management behaviors were observed across distinct demographic variables, such as educational attainment and disease types. These findings can serve as a reference for health authorities and community care centers to design health promotion programs for middle-aged and older adults, while also validating the theoretical applicability of the HBM to chronic disease self-management.
