概要:
「安寧緩和醫療條例」的修法與推動，強調醫療自主的重要性，尊重病人的意願，也減少家屬與醫護人員的負擔。但推動至今，簽署預立醫囑的人數有限；且研究對象多以醫護人員及住院病人為主。因此本研究以南投縣草屯地區中高齡者為研究對象，透過問卷調查收集資料，並以統計軟體spss12.0進行描述性統計、獨立樣本T檢定、單因子變異數分析和皮爾森積差相關；來瞭解中高齡者在預立醫囑知識、態度和經驗上的現況；並探討在不同背景變項下，知識、態度上的得分是否有差異；以及知識、態度和經驗三者間的相關性。結果顯示：（1） 草屯地區中高齡者預立醫囑的認知程度僅有中等水準，平均答對率為56.8％，對預立醫囑的態度偏正向，預立醫囑的經驗並不多；（2）在教育程度、職業、有無聽過安寧資訊等變項上，預立醫囑的知識有顯著差異；（3）在不同背景變項上，預立醫囑的態度未達顯著差異；（4）預立醫囑的知識、態度和經驗上，兩兩間無顯著的相關。透過有效的宣導方式，增進中高齡者對預立醫囑的認知，進而提升預立醫囑之態度，才能落實預立醫囑的理念。

"Hospice Palliative Care Act" emphasized the importance of patient's autonomy in medical care. But since the law was passed, only a limited number of people have completed an advance directive and researches that have been done focused mainly on medical staff and inpatients. Participants of this study are senior residents in Tsaotun, Nantou County. Questionnaires were used to collect data for the study and the following tests: descriptive statistics, an independent samples t-test, one-way ANOVA and Pearson's Correlation were run in SPSS 12.0 to understand the current status of the seniors' knowledge, attitude and experience about advance directives. Further, the study is to discuss if there’s difference in knowledge level and attitude of people with different background variables. The results are：（1）In Tsaotun area, the elderly showed a middle-level of knowledge about advance directives; their answers are 56.8% accurate; attitude toward advance directives was positive; and they have little experience of advance directives.（2）There’s significant difference in knowledge of advance directives by people with different education, occupation and awareness of advance directives.（3）There is no significant difference in attitude of people with different background variables, and （4）There is no correlation between any two of these variables: knowledge, attitude and experience of advance directives. By effectively promoting advance directives to increase knowledge about advance directives of the elderly first and thus enhance the attitude to the idea of implementing an advance directive.
