This study provides a conceptual framework that describes a relationship between agitation in dementia sufferers and the socio-physical environment of the long-term care setting. The proposed framework utilizes existing theory and empirical evidence from psychology, sociology, environmental gerontology, and to a lesser extent, biology. By unifying theoretical approaches prevalent in each one of the above domains, this integrative model will fill a gap in literature that has long concerned researchers in environmental gerontology.

The conceptual validity and integrative power of the framework, as well as potential impact of environmental design on elderly with dementia, is tested through a mixed-method study that involves simultaneous collection of both quantitative and qualitative data. The study combines a quantitative descriptive correlational design that involves secondary analysis of data from a study of eleven long-term care facilities in Southeastern Wisconsin, with a parallel qualitative descriptive case study of the same facilities. More specifically the design is of a concurrent nested strategy, where the qualitative method is embedded in the predominantly quantitative method during the data analysis phase.

The findings reveal notable patterns. Of particular interest is the discovery that temporal aspects of the socio-physical environment have a profound impact on agitation, when compared to physical features of the settings. Regulation of undesirable social and sensory stimulation, increased social contact, and structured daily activities are of paramount significance. Unfortunately, the presence of structured activities, staff-elicited activities implemented to meet therapeutic goals, was found to be all too rare within the long-term care settings investigated. The provision of subsidiary spaces of varying size and quality to decrease crowding, presence of single occupancy rooms, exclusion of a central nurses' station, decreased use of television, elimination of PA systems, use of sound absorbing materials, provision of balanced light sources, and absence of aversive odors were all inferred to benefit the long-term care environment.

The study concludes by identifying strengths and limitations of the proposed framework, and discussing its implications for future theoretical developments, research, and design applications.
