This paper reports findings from an analysis of data from 867 nursing home residents. Data were originally collected as part of the Nursing Home Case-Mix and Quality Demonstration (NHCMQ). It was a three step study. The first step was to conceptualize how the disablement process unfolds for residents and then match the Minimum Data Set with the Disablement Process model. The second step was to assess the measurement properties of the MDS ADL Self Performance Scale and the MDS cognitive scale. The third step was to understand functional trajectories over time and the factors that influenced these trajectories.

The MDS data set and the Disablement Process model were matched successfully. The MDS, however, did not have many of the required facility level characteristics. Both the MDS ADL self performance and the cognitive scales performed well as measuring devices and maintained their measurements properties across measurement periods. The average functional trajectory was showed a slight increase between admission and 3 months then a slight reduction from 3 months to 6 months. There was considerable individual variation around this group trajectory. The factors that influenced the trajectory were Alzheimer's, other dementia, arthritis, cerebrovascular accident, short/long term memory, balance, leg movement, trunk control, ROM hand/rist, tires during day, evidence of pain, cognitive skills for decision making, ease with doing planned and self initiated activities, establishing goals, pursues involvement in facility life, involved in group activities, stays busy with hobbies and sadness over lost role.
