Transitional care is an increasingly important component of health care due to the growing number of elderly, the impact of chronic conditions and the role of transitional services in managing these conditions. This study explored the participants' experience and effect of payer source in accessing and utilizing transitional services. The study design was cross sectional and retrospective utilizing quantitative and qualitative methods. Participants (n = 30) were interviewed regarding transitional care utilization and their perception of care. The sample was obtained from nursing home residents with a history of transitional care needs prior to admission. Factors measured included hospital care experience, sociodemographic measures, indicators of outcome, self-reported health status, community service utilization, family met needs, unmet needs and self-reported experience.

Results showed no significant differences in services accessed and utilized by payer source. Both groups reported lack of knowledge regarding services available, benefits and eligibility. Participants reported negative experiences in six areas of concern. Implications of findings included failure to utilize transitional services, lack of knowledge of transitional services, benefits and eligibility criteria; and unfamiliarity with chronic care needs. Potential studies include defining pathways to transitional services, creating venues for public education concerning transitional care, reform of public insurance programs and resources for patients and informal caregivers.
