Increasing numbers of nursing home residents are working-aged individuals between the ages of 18 and 64 years. Though there are several plausible explanations, it is unclear why working-aged adults are entering nursing homes in greater numbers. Recent data indicate that the prevalence of chronic disease and disability are increasing among working-aged adults. Combined, these trends may contribute to increased utilization of nursing home care. In addition to these factors, a lack of family and social supports may also contribute to increased use of nursing home care. One of the goals of this research is to determine the relationships between these variables and length of stay among working-aged nursing home residents. Further, increased nursing home use among this population is occurring within a post- Olmstead policy environment. According to the precedent set by the Supreme Court in 1999 in Olmstead v. L.C., individuals with disabilities are not to be indiscriminately placed in institutional settings when they are able and express a preference to live in the community. In other words, states are legally required to provide individuals with disabilities with access to community-based settings where appropriate and within the state's resources.

The purpose of this research is to determine the strength and direction of the relationships between length of stay and other variables among working-aged adults in nursing homes. To examine these trends, this study utilizes data from the National Nursing Home Survey (NNHS), survey years 1985 through 2004. Length of stay has fiscal and programmatic ramifications for the Medicaid program, as individuals with nursing home longer stays are more likely to transition to Medicaid. Additional policy implications relate to transition programs for adults already in nursing homes who may be better served in the community and home and community-based services for adults without familial and social supports. Lastly, this study suggests the need for policy interventions to improve the Medicaid program to ensure that working-aged adults with disabilities have viable options for accessing services.

This study adds to the existing body of research, because it (1) creates a profile of working-aged individuals entering nursing homes, (2) examines if this profile has changed between 1985 and 2004 using National Nursing Home Survey data, (3) determines the variables that have the greatest impact on length of stay, (4) estimates the relationship between discharge location and length of stay among discharge residents, and (5) examines the resulting policy implications for the Medicaid program.
