Depression among elderly persons is common. Primary care practitioners and psychiatric mental health professionals report that depression in the elderly often goes unrecognized and untreated. Major depression is disabling, highly prevalent, and adversely affects daily function and quality of life (Wei et al., 2005). Depression in the elderly can contribute to increased health care costs and medical co-morbidities. Delivering effective psychotherapeutic treatment to this population is important to decrease depressive symptoms, improve quality of life and prevent premature death/suicide.

Though research has demonstrated the effectiveness of bio-psychosocial treatments for elders with depression, twenty percent of this population does not respond to treatment. Thus, the purpose of this study was to gain insights from interviews with such elders that could contribute to improving treatment for depression in elders. Interviews were conducted with twenty depressed elders being treated with Interpersonal Psychotherapy (IPT) and medication. During the course of data analysis the following themes emerged: independence, spirituality, family, depression, medical co-morbidities and motivation. These themes enabled elders to perceive the meaning in their lives.

A perspective of the process of elders finding meaning in their lives was explicated such that characteristics of independence, motivation and spirituality enabled these elders to identify their need for treatment and to cope with their co-morbidities. Developing the role of the clinician as coordinator of health care and the appropriateness of motivational interviewing as a treatment strategy were presented as possible avenues to improving treatment for elders with depression.
