520 $a This study aims to explore the meaning of the quality of home care within the Quebecer socio-sanitary context. The quality of home care expressed by users, is still much neglected and almost out of institutional meaning in the Health and social system. The meaning of the quality of home care by users is not included in the Health and social system. We explore the impact of relationship system with the meaning of quality of home care by an elderly person, the formal ("visiting homemakers and case aids" )1 and informal and family caregivers. These agents are essential in the home support and theirs points of view are substantial in order to improve the quality of home care. Our goal is to show how the social representations of the quality of home care (AFS) according to the context of the home support. Our research is based on the social representation theory (Moscovici, 1961), which theory allows us to show the perceptions, the expectations and the attitudes with the quality of home care by each agent. It is a two years longitudinal study performed in Quebec with four triadic cases, each one including an old people, a formal caregiver and an informal and family caregiver.
520 $a We work especially with the Quebecers 65 and up with incapacities and using the AFS service and with the informal and family caregivers and formal caregivers ("visiting homemakers and case aids") from Health and Social Services Center. 24 in-depth semi-structured individual interviews were realized, 12 (4 cases including 3 agents) in the beginning of home care service and we reiterated interviews after several months of use. Theirs points of view and theirs experiments help us to catch the link between the realities of home support and the quality of home care. We follow these 4 cases and detail the context.
520 $a Results show that the elderly people and informal and family caregivers have a social representation of quality of home care highly related to the context (perceptions and practice), but it is different by the AFS. The social representation of the quality of home care by AFS depends both on the organisational structure of Health and Social Services Center (rules improving the security) and on the AFS group. AFS's representations are professional representations. Ethical issues of the quality of public home care are also discussed such as the respect, the role, the participation and the knowledge of each agent in the home support.
