摘要註:	由於台灣已邁入高齡社會，再加上少子化效應，家庭結構改變，高齡者照顧需求增加，機構照顧將逐步被大眾接受且個案需求量增多，然而，傳統的尿布、約束、臥床的機構照顧模式已不符生活照顧品質的要求，因此，台灣於2011年引入日本自立支援照顧模式，符合被照顧者的照顧需求，過程中家屬扮演重要的推動支持角色，本研究採用質性研究法，針對五間實施自立支援照顧模式的機構住民家屬進行訪談，使用半結構式訪談法，主要訪談家屬在進行自立支援照顧初期、過程及個人必須如何與機構配合，如何支持自己的親屬進行有效的自立支援照顧，彙整訪談分析結果如下：（1）初期最重要的是與機構共同討論自立支援照顧計畫內容，越清楚明白照顧計畫，將有助於住民的生活品質提升；（2）過程中家屬配合推動意願越高，與機構互動頻繁者，對住民行動與心靈支持都有很大幫助；（3）多陪伴及配合照顧計畫多鼓勵親屬進行獨立自主能力回復訓練，提升生活品質，有助於推動自立支援照顧成效。本研究結果可提供機構引入自立支援照顧模式時尋求家屬高度配合提升照顧生活品質之參考。

However, the old system of care, which greatly focuses on the use of diapers, physical restrictions, and physical restraints, is no longer an option consistent with the expected improvement in quality of care. To meet those expectations, Taiwan has adapted the Japanese self-supporting care model since 2011, in which family members play a major supporting role. This research uses qualitative research method, interviewing five families that have chosen this residential care system for their elderly, and implements semi-structured interview, while focusing on the cooperation process from throughout the preliminary stage to implementation, and the attitudes that allow the families to effectively support the elderly family member.The results:（1）The most important factor in the preliminary stage is effective communication with the staff on each detail of the self-supporting care model, which may raise the quality of living for the elderly resident.（2）Family members that are highly cooperative, and have frequent interactions with the institution, allow the elderly resident greater mobility, and give them the most psychological support.（3）Family members that provide frequent companionship with the elderly member, while fully cooperating with the established model, encourages the elderly member to return to an independent lifestyle, thereby increasing the quality of life, and the effectiveness of the self-supporting care model.This research can be implemented in long-term care institutions that seek cooperation from family members in order to increase the quality of life for their residents.
