摘要註:	2015年12月通過《病人自主權利法》，並於2019年1月施行，透過預立醫療照護諮商（Advance Care Planning ， ACP）後再做預立醫療決定（Advance Directives ， AD），希望年滿20歲具完全行為能力之成年人，可經過專業醫療團隊諮詢後，書面立下拒絕維生治療與接受相關醫療照護之指示，當進入特定醫療臨床情境時，依照醫療指示意願給予適切的醫療照護。然而，預立醫囑大部分都在死亡將近時才作出決定，因此本研究以南投市中高齡者為研究對象，透過問卷調查收集資料，並以統計軟體進行描述性統計，來瞭解中高齡者在預立醫囑選擇意願、接受度之現況；並探討南投市中高齡者未來簽署預立醫囑的理由。結果顯示：（1）南投市中高齡者預立醫囑選擇意願中因為疾病無法自行照顧時結果顯示，有近六成的受測者不願意到醫院及安養機構入住，與近六成的受測者選擇在家裡安詳辭世。另針對後事的安排，以火化後放入靈骨塔為最多占67.0%；（2）在預立醫囑接受度上，整體量表之平均數為28.7分，顯示中高齡者對於預立醫囑的接受程度有近八成的認同；（3）願意簽署預立醫囑主要的理由為，有尊嚴並減輕痛苦（85.7%）；減少金錢支出和家人負擔（74.1%）則為次之。希望未來透過有效的宣導方式，增進中高齡者對預立醫囑的認知，加強醫病間溝通，並協助完成自主善終權利。
