摘要註:	依勞動部2021年2月統計外籍移工在台人數共計712,107人，其中從事看護工約34.8%。而在雲林地區的看護工以越南籍最多，佔44.48%。外籍看護工照顧失智長者過程，比起照顧失能長者，因文化差異及語言障礙的照顧問題，更突顯研究此問題的重要性。本研究以質性研究方法為主，以雲林縣某三家混合型長期照顧服務機構越南籍看護工爲研究對象，以立意取樣訪談5位越南籍看護工，探討機構外籍看護工照顧失智症個案之心路歷程。研究結果發現：1.沒有足夠的培訓，造成照顧上專業知識不足，尤其面對失智的問題行爲，以游走與妄想最爲困擾。2.照顧壓力支持系統缺乏；隨著失智症長者病程變化，溝通上越來越不容易，溝通障礙會造成外籍看護工照顧上的壓力與困境。3.因不懂失智症長者照顧技巧，相關照顧議題的因應大多以內部資源為主，處理方式會先尋求現場護理人員的協助。4.越南籍看護工在照顧過程中，最常遇到的是文化差異中的語言障礙，外籍看護工彼此間會相互協助與鼓勵。5.越南籍看護工運用非正式資源作為情緒之處理方式，每當有照顧上的壓力及思鄉的負面情緒時，先壓抑下來，以親朋好友為宣洩管道。建議政府及長照機構管理者應先導入失智症照顧服務專業課程給予外籍看護工，之後可以增加外籍看護工同儕照顧支持系統、資訊提供及心理支持，除了可以維護及提升失智症專業照顧的品質，同時兼顧到輔導協助外籍看護工在失智症專業照顧上的困境與障礙。

Compared with caring for the disabled elderly, it is more difficult for foreign nurses to take care of the disabled elderly because of cultural differences and language barriers. This study is based on qualitative research method, taking foreign caregivers from three long-term care institutions with blended forms in Yunlin County as the research objects; We use the purposive sampling method to do in-depth interviews with 5 Vietnamese care workers and try to explore the process of mental and psychological adaptation and experiences to care for the elderly with dementia. The results of the study found as following, 1. There were insufficient training education system and knowledge about care to dealing with dementia problems, especially in the most troublesome wandering and delusions. 2.Following with nature course of dementia, the migrant caregivers have been facing a lot of stress and ordeals at being more increasingly difficult for communication with the elders. 3. Due to lake of care skills about the elders with dementia, the migrant caregivers will use internal resources to handle first and call for help from in charged nurse first. 4. The language barriers of cultural differences are most often encountered for Vietnamese migrant caregivers They will assist and encourage each other. 5. Vietnamese migrant caregivers use informal resources as a way to deal with emotions. When migrant caregivers have suffered from caregiving and negative feelings of homesickness, they will suppress emotions first and complained to their relatives and friends by the phones later on.The migrant caregivers have not received any tailored-made training of care-giving before going to Taiwan, therefore, they just carry on caring by primitive and old ways. That could not only diminish quality of care-giving but also cause overload on physical and mental of migrant caregivers. Through this study, we understand the behavior, coping skill and the mode of adaptation of migrant caregivers and how they fit in life out of their hometown were when coping with the elderly with dementia. Therefore, this study suggests the government and the supervisors of long-term care institution should provide tailored-made curriculum of dementia care service to migrant caregivers in advance to maintain and improve the care quality for dementia.
