摘要註:	自立生活的核心觀念生活著（alive）與自我決定（self-governed），致力於提高年長者日常生活之活動表現，增加年長者的活動量，減少對於支持和協助之依賴。基本上，自立支援較適用於機構式照顧模式，但是在居家照顧規模日益擴增之際，自立支援是否能挪移到居家照顧領域，機構實施自立支援與居家照顧服務的環境差異為何，包含家屬的反應會如何，以上動機遂構成本論文之問題架構。本研究主要探討自立支援照顧模式導入居家照護之影響因素，研究目的為探索長者本身之認知及其過去的經驗，以及家庭照顧者是否影響長者對於自立支援模式之影響，健康狀況是否參與及如何影響自身對於自立支援模式之接受。本研究採面對面訪談為主，參與觀察為輔，研究對象為申請居家照顧的長者，研究參與者人數為三十位。研究結果為：（1）長者的認知影響其對自立支援的接受程度；（2）長者的健康狀況影響其對自立支援的接受程度；（3）家庭照顧者是間接影響長者對於自立支援模式接受的因素；同時也發現上述三者之外的因素介入自立支援照顧模式之移入居家照護。

Basically, self-reliance support is more suitable for the institutional care model, but when the scale of home care is increasing, can self-reliance support be transferred to the field of home care？ How will the response be, the above motivations constitute the problem framework of this paper. This study mainly explores the influencing factors of the introduction of the self-reliance support care model into home care. The purpose of the research is to explore whether and how the elderly's own cognition and past experience, as well as the environment of home service, affect their acceptance of the self-reliance support model. Participatory observation and interviews were used in this study. With interviewing and participant observation, this study shows up certain factors, more than original problem design, that influence the transition of self-supporting model into home care.
