摘要註:	老年人罹患疾病後往往需要較長的恢復期以回復之前的生活功能狀態，透過中期照護可提供急性醫療與慢性照護間的橋樑。本研究旨在探討於民國100年1月至101年12月期間，社區醫院186位65歲以上銀髮族，接受中期照護前、後，對其周全性老年評估、生活品質與健康狀態之成效及其相關影響因素。本研究採次級資料分析法，研究結果顯示個案接受中期照護服務後，在周全性老年評估結果的各個面向，均有顯著的臨床進步；在生活品質評估及健康狀態方面，亦有明顯的進步；而在探討個案基本屬性方面則發現，年齡、婚姻狀況與身份別對工具性日常生活功能，個案的教育程度對心智狀態，以及年齡與營養評估亦有影響，證實中期照護是具有明顯成效的服務模式。根據研究結果，提供以下建議：（1）對機構而言，{鴽開床照護計畫的擬定應考慮個案基本認知程{翵及家庭的支持參與；未來中期照護發展模式除了善用現有醫療與長期照護體系資源外，應與各專業人員團隊合作，經由完整的系統性評估與治療，將資源做最有效率的整合運用。（2）對學界而言，建議往後的研究可進一步探討個案返家後的長期成效評估；探討個案基本屬性對中期照護成效之影響；亦可在接受與不接受中期照護之成效差異做比較，或對於中期照護之健保經濟效{翵方面多做研究分析。（3）對政府而言，在推動中期照護模式時，應對各醫療專業人員及輔助人力配備有所規範，且建立合理的付費制度，以維護中期照護之品質。

Therefore, through the development of Intermediate Care, we can provide the bridge of acute medical care and chronic disease care. The purpose of the study was to survey the effectiveness and the factor of the 186 elders over 65 years old around receiving intermediate care of comprehensive geriatric assessment, quality of life and health status of the effectiveness of its related factors for a community hospital from January of 2011 to December of 2012. Secondary deta analysis, the results show case into the intermediate care services before and after comprehensive assessment of the various age-oriented showed a significant clinical improvement, assessment of quality of life and health status are also significant improvement. In the basic properties of cases are found, age, marital status and identity category on instrumental activities of daily living , level of education of cases have an impact on the mental state, and influence of age and nutritional assessment. Intermediate care is certainly a remarkable service model. According to the study results provide the following suggestions : (1) For the institution, clinical care plan intended to consider the case of basic cognitive attainment and family support; in the future the development of intermediate care should utilize the existing health care system and long term care system resources and cooperate with the team of professionals, through a complete systematic assessment and treatment make the resources having the most efficient use of integration. (2) For the academic, recommend future research to further explore the case of long-term effectiveness evaluation after discharge; study of the basic properties of the intermediate care of the effectiveness of care; compare the effectiveness of the differences between accept or do not accept intermediate care, or do more research and analysis of intermediate care for the health economic benefits. (3) For the government, when promoting the intermediate care model should standardize the response to various medical professionals and associate human, and building a reasonable payment system in order to maintain the quality of intermediate care.
